Click on the online application link to complete the MiniCo application and submit it electronically.

NOTE: When you select "Online Application" and submit an application electronically, you will receive a confirmation e-mail from MiniCo one business day following submission. If you do not receive a confirmation e-mail, please contact MiniCo as soon as possible at 800-447-8383.

Click on the Print/Fax application link to download and print the MiniCo application. Please fax to 602-861-1094.

Now MiniCo can accept your ACORD application accompanied by this short supplemental form. Click on the link to download and print the ACORD supplemental application.

Once your completed application is delivered to the MiniCo Underwriting Department by e-mail, fax or on-line, your designated underwriter will contact you. if you have any question about submitting an application, please contact MiniCo’s Customer Care Department at 800-447-8383 or e-mail customercare@minico.com.
